City will identify the key contact (name, address and <br />phone number) at the selected cities, execute the transmittal letter and transmit the <br />documents directly to the key contact at cities to be surveyed. <br /> <br />Task 2: After the distribution of the survey instrument, HF&H staff will follow-up with <br />one phone call to each contact to determille that the instrument has been received and <br />respond to questions. One week after the submission date of the completed surveys, <br />HF&H staff will make a second call to the contact at those cities that have not <br />responded. <br /> <br />Task 3. Two weeks after the second call, HF&H staff will compile the data from the <br />responses received and prepare a brief report of the findings including a tabular listing <br />of the responses. <br /> <br />Task 4.
