ORIGINAL' <br /> <br /> AMENDMENT TO AGREEMENT <br /> CDS GROUP 112105 <br /> <br /> AGREEMENT dated Pebruary ls 197#~ as amended~ between CITY OF REDWOOD CITY~ and <br />CALIFORNIA DENTAL SERVICE is hereby further amended~ effective April Is 1977~ as follows: <br /> <br /> Paragraph I-A is amended to read: <br /> <br /> A. Monthly Payment: The sum of $19.71 for each eligible employee in Unit ! and the sum <br />of $2#.26 for each eligible employee in Unit 2. <br /> <br /> Paragraph II-E is amended to include: <br /> <br /> Effective April 1~ 1977~ the deductible shall not apply to services provided to eligible <br />employees and dependents in Unit 2. <br /> <br /> The first paragraph of Eligibility Rules~ Appencfix "A"~ is amended to read: <br /> <br /> Effective AprLl 1~ 1977~ eligible employees are defined as all regular and regular part-time <br />employees employed on a regular and continuous basis except casual and temporary employees. <br />Such employees will become eligible on the first day of the month coincident with or next following <br />their date of hire. All full-time unrepresented employees and ali full-time and regular part-time <br />employees in Service Employees International Union Local #715 shall be included in Unit 2. <br /> <br /> Eligibility Rules are amended to include: <br /> <br /> Coverage for employees and their eligible dependents on an authorized leave of absence <br />shall be continued for a maximum of ninety (90) days. <br /> <br /> DATED: April 1~ 1977 <br /> <br />ClT O];~REDWOOD C~TY: .
