APPEND'IX '~D" ORTHODONTIC ~ENEPIT RIDER In considerati~n of the payments specified in paragraph I of t~e =ttachecl A~reement, and subject to all of the terms ~ conditions thereoft except es herein otherwise speclfled~ CDS asr&es to provide Orthodontic Benefits to eliitlble patients, es foUows~ I. Orthodontics are defined as the procedures performed by a licensed dentL~ts~ [nvolvir{ the use of an actl~e orthodontic appliance and post-treatment retentive aq~4iances for treatment of rnadaliff~ment of teeth and/or jaws whtch sJ~flca~ltJy b-tterfere~l with their function. 2. CDS will pay or otherwise discharge 60% of the lesser of the usual, custom&r), and reasonable fees or tf~e fee~ actually ch~r~d for Orthodontics, provided that the amount p&yable to corresponding services set forth in the Orthodentlc Table of Allo~vancos, a copy of which is attached hereto, marked E~xhiblt 1 and lncorp0~ted herein by reference. All payments sh~ll be on a monthly or periodic baslst in acc~dance w~th the de~ti~t~ normal blllin~ practice. 3. The maximum amount payable b~ CD$ far all Orthodontics rendered to each eligible dependent child shall he ~00.00~ and the limitatiorm on maximum emotmts payable du~-tr~ a calendar year~ if any, specified in the attached Agreement, shall not apply to C~thodontics, 6. EXCLUSIONS AND LIMITATION~ In addition to the J[xclusions and Limitations stated tn Appendix C to the attached Agreemen% the followin8 exclusions and Umitation$ shall apply to Orthodontic t~eneflts, (a) The obligation of CDS to make monthly or other periodic payments for an Orthodontic treatment plan shall cease upon termination of treatment for any re&sort prior to completion of the case. (b) The obllsatlon of COS to make monthly or other periodic payments for an Orthodontic treatment plan be&mt prtor to the elllJlbllity date of the patient shall commence with the first payment due following the patient~s eligibUity date. The above-mentioned maximum amount payable will apply to th~ and subsequent payments, (c) CDS will not rr~ke any payment far repair or re~acement of an Orthodontic appliance furnished, tn whole or in par% under this pro;ram. (d) CD~ obligation to rn~ke monthly or other periodic payments for Orthodontics shall terminate on the payment due date next following the date the patient 1o~ eligibility er on the termination date of the Agreement, whlche~or shall occur first.
