APPENDIX "A" <br /> <br /> ELIGIBILITY RULES <br /> <br /> Effective February 1, 1974, all present, permanent employees of <br />CITY OF REDWOOD CITY are eligible under the Dental Care Service <br />Agreement. Ail future permanent employees will become eligible <br />on the first day of the month coincident with or next following <br />their date of hire. <br /> <br /> The dependents of eligible emRloyees are eligible under the <br />Dental Care Service Agreement. Dependents are lawful spouse <br />and unmarried dependent children to 19 or to 23 if enrolled as <br />full-time student in an accredited school, college or university. <br />Children include step-children, adopted children and foster children, <br />provided such children are dependent upon the employee for support <br />and maintenance. <br /> <br /> An unmarried child 19 years or over may continue to be eligible <br />as a dependent if he is incapable of self-support because of <br />physical or mental incapacity that commenced prior to reaching age <br />19, provided a physician's certificate is submitted within six months <br />following his 19th birthday or the effective date of this Agree- <br />ment. <br /> <br /> Dependents in military service are not eligible. <br /> <br /> Eligibility of employees shall terminate on the last day of the <br />month in which full time employment has terminated. Dependents <br />shall remain eligible until the last day of the month coincident <br />with or following termination of eligibility of the employee or <br />loss of dependent status, whichever shall occur first. Eligibility <br />shall, in any event, terminate immediately upon termination of this <br />Agreement. <br /> <br /> <br />
