ARTICLE II - ELIGIBILITY 2.02 Applicant shall permit CDS, by its auditors or other authorized representatives, on reasonable advance written notice, to inspect records of Applicant in order to verify the accuracy of lists of Eligible Employees prepared by Applicant and submitted to CDS. 2.03 Every employee and dependent who meets the following conditions of eligibility shall be considered an "Eligible Person", and the appropriate monthly payment is made pursuant to Article III of this Contract, for the month(s) in which covered dental services were provided. ~p~ F_~fective March 1~ 1975~ Fligible Employees are defined as all regular-~ -~ employees employed on a regular and continuous basis except casual and temporary employees. Such employees w'fl[ become eligible on the first day of the month coincident with or next following their date of hire. All full-time unrepresented employees and all ful~-~l~me and regular part-time employees in Service Employees International Union ~ ~15 shall be included in Unit 2. The Dependents of Eli~ble Employees are eligible on the first day of the month coincident with the employ~ or~, .thereafter, immediately upon attainment of dependent status. Dependents ar~ lawful spouse and unmarried dependent children from birth to age 19 or to 23 if enrolled as full-time student in an accredited school~ college or university. Children include s~ep-children, adopted children and foster children, provided such children are dependent upon the employee for support and maintenance. An unmarried child 19 years or over may continue to be eligible as a dependent if he is incapable of self-support because of physical or mental incapacity that commenced prior to reaching age 19 and if he is chiefly dependent on the Eligible Employee for support and maintenance, provided proof of such incapacity and dependency is submitted within thirty-one (31) days ~ter a request therefor by either CDS or Appllcant~ and subsequently as may be required by either CDS or Applicant, but not more frequently than annually aster the incapacitated and dependent child has attained age 21. Dependents in military service are not eligible. 2.05 Eligibility of employees shall terminate on the last day of the month in which full- time employment has terminated. Dependents shall remain eligible until the employee's eligibility terminates or until loss of dependent status whichever shall occur first. Eligibility shall, in any event~ terminate immediately upon termination of this Contract.
