<br />I--U <br /> <br />___un - ~ <br /> <br />-~---- <br /> <br />ARTICLE -- 1 DEFINITIONS <br /> <br />These terms, when used in this Contract, mean the following: <br /> <br />1.1 <br /> <br />1.2 <br /> <br />1.3 <br /> <br />1.4 <br /> <br />1.5 <br /> <br />1.6 <br /> <br />1.7 <br /> <br />1.8 <br /> <br />1.9 <br /> <br />"Attending Dentist's Statement" is the form completed by the Dentist to <br />request Delta's payment for dental services or predetermination for <br />proposed dental treatment. <br /> <br />"Benefits" means those dental services which are available under the terms <br />of this Contract as set out in Article 4. <br /> <br />"Contract" means this agreement between Delta and City of Redwood City, <br />including the attached appendices. This Contract is the entire Contract <br />between the parties. <br /> <br />"Contract Term" means the period beginning on February 1, 1998 and <br />ending on January 31, 1999, and each subsequent yearly period during <br />which this Contract remains in effect. <br /> <br />"Dentist" means a duly licensed Dentist legally entitled to practice dentistry <br />when and where services are provided. <br /> <br />"Eligible Dependent" means any of an employee's dependents who are <br />eligible for Benefits under Article 2 of this Contract. <br /> <br />"Eligible Employee" means any employee who is eligible for Benefits under <br />Article 2 of this Contract. <br /> <br />"Eligible Person" means an employee or a dependent who is eligible for <br />Benefits under Article 2 of this Contract, or a person ceasing to meet such <br />conditions who chooses continued coverage as set out in Article 10, and for <br />whom Delta receives the appropriate monthly payment set out in Article 3. <br /> <br />"Eligibility Date" means the date an employee's eligibility for Benefits <br />becomes effective under the terms of this Contract. <br /> <br />2 <br /> <br />-------- <br />
