<br />~---- ---- <br /> <br />10.4 <br /> <br />10.5 <br /> <br />10.6 <br /> <br />10.7 <br /> <br />~---- <br />~ -- ----~ <br />u_- ~-~-~ -----~ -- <br />~____n <br />----~ <br /> <br />Eligible Dependents who choose to continue their coverage based on <br />Qualifying Event 1O.1(a), described above, and for whom a second <br />Qualifying Event [but not 1O.1(a) or (f)] occurs within the period of <br />continued coverage may choose to continue their coverage for a maximum <br />of 36 months following the month in which the first Qualifying Event <br />occurred [in the case of a second Qualifying Event described in 1O.1(b), <br />(c), or (e)], or for a maximum of 36 months following the month in which <br />the second Qualifying Event occurred [in the case of the Qualifying Event <br />1O.1(d)]. <br /> <br />Eligible Dependents whose coverage under this program ends due to <br />Qualifying Events 1O.1(b), (c), (d), or (e), may choose to continue their <br />coverage for 36 months following the month in which the Qualifying Event <br />occurs. <br /> <br />Eligible Persons whose coverage under this program ends due to Qualifying <br />Event 1O.1(f) may choose to continue their coverage until death (in the case <br />of a retired Eligible Employee), or for 36 months after the date of death of <br />the retired Eligible Employee (in the case of Eligible Dependents of a <br />retired Eligible Employee). <br /> <br />Continued coverage can be chosen only by notice to City of Redwood City, <br />which must be given no later than 60 days after a termination of coverage <br />by reason of a Qualifying Event, or within 60 days after the Eligible Person <br />receives a notice from City of Redwood City about his or her rights to <br />continued coverage because of the particular Qualifying Event, whichever <br />is later. Persons for whom a Qualifying Event described in 10. 1 (c) or (e) <br />occurs must report it to City of Redwood City within 60 days, or lose their <br />right to choose continued coverage. <br /> <br />29 <br />
