Brief physician advice <br />regarding modifications of these risk factors is given to the employee during the pre-employment <br />physical exam. A "prescription for prevention" details cancer screening tests that are now due or <br />past due so that the employee can present it to his or her new doctor after enrollment in a health <br />plan. The physician encourages employees to discuss the lifestyle risk factors that have been <br />identified with their new doctor. They are also given information about their employee assistance <br />program at work as well as community resources. Each employee receives a Healthw;se <br />Handbook and a brochure describing health education classes at Kaiser and on the job. <br /> <br />* Lifestyle risk factors are evaluated while protecting the privacy rights of each employee - <br />employees are asked not to write their names on the risk factor questionnaire. The employer's name <br />is requested and questions are numbered so that the prevalence of specific risk factors among <br />employee groups may be evaluated for use in planning lifestyle risk reduction programs. <br /> <br />,.- <br /> <br />H'T--"'--' T <br />
