<br />EXHIBIT "An <br /> <br />SCOPE OF SERVICES <br /> <br />Inspect trees as designated on the Tree Preservation Ordinance permit applications. <br />Output shall conform to RFP dated May 19, 1998. That RFP is herein incorporated by <br />reference. <br /> <br />Two levels of service shall be provided, determined by whether the application is for tree <br />removal or tree pruning. <br /> <br />For each permit, the following information shall be provided to the City based on the on- <br />site inspection: <br /> <br />A) <br /> <br />B) <br /> <br />Tree Removal Permit Applications: <br />1. "Site Observation Checklisf' recording the tree's species, size <br />(trunk diameter measured between 6" and 36" above grade), and <br />tree and site conditions pertinent to the subject application. <br />Photograph of the tree (either Poloroid film or 35mm film). <br />Hand sketched site drawing, "not to scale," showing the tree, <br />major structures arid other pertinent information to the site <br />conditions. <br />Written "Inspection Report" with recommendations to the City for <br />approving or denying removal, stating the basis for such <br />recommendation. <br />Recommendation that the applicant plant a replacement tree <br />when appropriate for the permit conditions and available spacing <br />on the property. <br /> <br />2. <br />3. <br /> <br />4. <br /> <br />5. <br /> <br />Tree Pruning Permit Application: <br />1. Written "Inspection Report" recording the tree's species, size <br />(trunk diameter measured between 6" and 36" above grade), and <br />tree and site conditions pertinent to the subject application. <br />Include recommendations to the City for approving or denying the <br />proposed pruning, stating the basis for such recommendation. <br />Provide recommendations for performing the pruning task within <br />ISA Pruning Guidelines (or ASCA Pruning Guidelines in the case <br />of Palm trees). <br />If recommending denial, or where greater detail would be <br />important, a "Site Observation Checklist" similar to the material <br />provided above in number 1 for removal applications. <br />If recommending denial, or where greater detail would be <br />important, one photograph (either Poloroid film or 35mm film) of <br />the tree. <br />Hand sketched site drawing, "not to scale," if it would reduce <br />ambiguity or provide needed detail. <br /> <br />2. <br /> <br />3. <br /> <br />4. <br /> <br />5. <br /> <br />Ray Morneau shall personally make the on-site inspections and write the inspection <br />reports, unless a substitute person is submitted to the City in a written request and the <br />request approved <br /> <br />,.- <br /> <br />" r-------T <br />
