<br />COUNTY OF SAN MATEO <br />PERSONAL SERVICES AGREEMENT MEMORANDUM <br />RECOMMENDATION FOR EXECUTION OF <br />AN AGREEMENT WITH INDEPENDENT CONTRACTOR <br />TO <br />Margaret Taylor, Director, Health Services <br />Health Services Administration <br />Charge to Budget Unit 55137 <br />FROM <br />SUBJECT: Independent Contractor City of Redwood City, Redwood City 2020 Program <br />It is recommended that the Health Services Director authorize the attached Agreement with Independent <br />Contractor. The requirements of Administrative Memorandum D-4 and the Purchasing Division Customer Guide <br />have been met. <br />Briefly describe work to be accomplished (describe in detail in Exhibit "A" of the Agreement; include beginning <br />and ending dates for the contract). WORK MUST NOT COMIVVII;NCE UNTIL THE HEALTH SERVICES <br />DIRECTOR SIGNS THE CONTRACT. <br />o Work to 6e Accomplished: <br />Contractor shall work with Redwood City Family Centers to provide outreach, enrollment and retention <br />activities to reach San Mateo County children and families who are eligible for but not enrolled in <br />health coverage. The Contractor will work in partnership with other community agencies, .community <br />institutions, businesses, and civic organizations to increase awareness of the Children's Health <br />Initiative. The Contractor will partner with Health Services' Community Health Advocates and Human <br />Services Agency eligibility staff to distribute general health and Medi-Cal/Health Families and Healthy <br />Kids information at community sites in the Redwood City area of the County and enroll eligible <br />families in Medi-Cal, Healthy Families and Healthy Kids. The Contractor will also assist in planning <br />and organizing mass health enrollment events in the Redwood City area of the County. <br />o Reason for Selection: <br />The Contractor is a community collaborative that works closes with Redwood City Family Centers to <br />provide integrated family support services. In conjunction with the Family Centers it has always offered <br />health insurance outreach, enrollment and retention services to families in the poorest areas Redwood <br />City, East Redwood City and unincorporated North Fair Oaks. The funding from this grant will expand <br />the current activities. <br />o Amount and Source of Payment: <br />Contractor will be paid a maximum of TWENTY THOUSAND DOLLARS ($20,000). The funding <br />will come from the Children's Health Initiative. There is no net County cost. <br />o Term of Contract: <br />January 1, 2002 to December 31, 2003 <br />
