Schedule A <br />City of Redwood City, Redwood City 2020 Program <br />1. Contractor shall work with Redwood City Family Centers to meet the following performance targets: <br />a. Increase 100 children, youth, and families' awareness, understanding, and perception of Medi-Cal, <br />Healthy Families, and Healthy Kids (MC/HF/HK) per month through such activities as information <br />dissemination and presentations. <br />b. Plan and organize at least 2 mass health enrollment events in the Redwood City area of San Mateo <br />County. <br />c. Assist in enrolling 320 children into Medi-Cal, Healthy Families and Healthy Kids. <br />d. Re-enroll 60 children into Medi-Cal/Healthy Families. <br />?. Contractor shall work with Redwood City Family Centers to provide the following services: <br />a. Inform children and families regarding HF/MC/HK by distributing and presenting culture specific health <br />education and coverage materials through existing community networks, for example schools and other <br />community based organizations in Redwood City area of San Mateo County. <br />b. Develop an updated outreach and enrollment work plan by January 3, 2003. <br />c. Implement a vigorous enrollment plan families living in the Redwood City area of San Mateo County. <br />d. When key staff is not available for enrollment assistance, Contractor will refer eligible individuals to the <br />Health Services Community Health Advocate ensuring that the clients receive health coverage. <br />e. Use requested paper based and web-based application and enrollment tracking forms developed for the <br />Children's Health Initiative. <br />3. Contractor shall ensure that appropriate staff attend and participate in the following contract related meetings and <br />complete the following reporting requirements: <br />a. Key staff for HF/MC/HK outreach will meet with Health Services' HF/MC/HI~ Outreach coordinator <br />every t<vo months for on-going training ott HF/MC/HK outreach and enrollment activities. <br />b. Every quarter, the Children's Health Initiative Project Director and the Contractor's HF/MC/HK project <br />supervisor will also attend meetings to receive a quarterly progress report. <br />Key staff for HF/MC/HK outreach will attend the Community Based CHI Coalition meetings in order to <br />respond to evolving enrollment, coverage and retention issues. <br />d. Contractor shall provide a report each month of the number of children that have been reached through <br />dissemination of outreach materials, screened for eligibility, assisted with HF/NIC/HK enrollment, and <br />referred to Community Health Advocates. <br />
