<br />t~ATEMENT SERVICES AGREEMENT <br />------- ------- <br /> <br />DUPLICA TE <br />OHIGI NAL <br /> <br />("Digtricttl) <br /> <br />and CITY OF <br /> <br />The SAN MATEO COUNTY MOSQUITO ABATEMENT DISTRICT <br />REDWOOD CITY ("Owner") agree as follows: <br /> <br />2 . <br /> <br />3. <br /> <br />1 . <br /> <br />The District will perform abatement services for the Owner's problem <br />site as described below. The District will. as reasonably necessary. <br />develope and implement mosquito ßurveillance and control activities <br />with its own personnel. materials and equipment at the problem site. <br /> <br />Problem site: the catch basin storm drain <br />developed areas of Redwood Shores. <br /> <br />system in the presently <br /> <br />Owner shall reimburse Distsrict at a rate of $11.50 per hour. not <br />exceed 200 man hours per year. The District will bill Owner for <br />actual services rendered. Owner will pay for such services within <br />30 days of receipt of each bill. <br /> <br />to <br /> <br />4. <br /> <br />The occurrence of any unusual problems thast would require addi- <br />tional manpower. equipment and materials. will necessitate a change <br />order as an addendum to this agreement. It is understood that any <br />changes would be mutually agreed to prior to the initiation of the <br />order. All additional costs related to change orders would be <br />negotiated separately. <br /> <br />5 . <br /> <br />This agreement shall be effective <br />1986.
