<br />ORIGINAL <br /> <br />AGREEMENT <br /> <br />for <br /> <br />HEALTH AND WELLNESS PROGRAM MODIFICATIONS <br /> <br />(YMCA OF THE MIDPENINSULA, SEQUOIA BRANCH) <br /> <br />-a-- <br />THIS AGREEMENT, made and entered into this 3° day of <br /> <br /> <br />~ ,1998, by and between the CITY OF REDWOOD CITY, a <br /> <br /> <br />municipal corporation of the State of California, (City), and YMCA OF THE <br /> <br />MIDPENINSULA, SEQUOIA BRANCH, a California non-profit corporation <br /> <br />(Contractor); <br /> <br />WITNESSETH: <br /> <br />WHEREAS, Contractor conducts a health and wellness program for <br /> <br />residents of the City (the Program); and <br /> <br />WHEREAS, Contractor has requested financial assistance in order to <br /> <br />install modifications to remove architectural barriers in facilities used by residents <br /> <br />with disabilities of City during the fiscal year commencing July 1, 1997, and <br /> <br />ending June 30, 1998; and <br /> <br />WHEREAS, City has reviewed said request and desires to allocate to <br /> <br />Contractor the sum of Fifty Thousand and no/100 Dollars ($50,000.00) from its <br /> <br />Community Development Block Grant funds for the Program; <br /> <br />NOW, THEREFORE, the parties hereto agree as follows: <br /> <br />1. Financial Assistance. City shall allocate to Contractor the sum of <br /> <br />Fifty Thousand and no/100 Dollars ($50,000.00) for exclusive use by Contractor <br /> <br />Agreement-4 <br />4/27/98 <br /> <br />1 <br /> <br />.- '" <br />¡ <br />
