Information <br />and referral empower people to make the right choices to live <br />more inde endentl . <br /> <br />Performance Indicators <br />At closure of service, the <br />Service Provider will ask the <br />consumer if they are more <br />independent due to CID's <br />services <br /> <br />Consumer surveys sent to <br />each resident will ask if <br />information led to more <br />inde endence <br /> <br /># of Unduplicated RWC Residents/Households to be Assisted (Only programs serving <br />individuals may report individuals.
