EXHIBIT A <br />CITY OF REDWOOD CITY <br />Fresh Lifelines for Youth, Inc. <br />FISCAL YEAR 2017-2018 <br />Agency Information <br />Agency Name: Fresh Lifelines for Youth, Inc. <br />Address: 330 Twin Dolphin Drive, Suite #109 <br />City, State, Zip: Redwood City, CA 94065-1408 <br />Activity/Program Title: FYI Law and Leadership Programs for Redwood <br />City Youth <br />Cluantitative Goals <br />Description of Services to be Provided: <br />FLY will provide its Law Program to between 30 and 60 at -risk Redwood City <br />youth. FLY will provide the Leadership Program to a minimum of 4 <br />moderate/high-risk Redwood City youth. The Law Program is an innovative law <br />education course taught 2 hrs/week over 12 weeks by trained volunteers and <br />FLY staff. After the Law Program, youth assessed as moderate to high-risk and <br />in need of support to get them on the path to success are eligible to join the <br />Leadership Program - 10 months of intensive 1:1 case management, pro -social <br />group events and community service with their peers. <br />Primary Quantitative Objectives: <br />FLY's Law Program will serve at least 30 incorporated Redwood City persons <br />(youth). <br />NOTE: The grant performance period begins July 1, 2017 and concludes June 30, <br />2018 <br />REV: 09-06-17 RL <br />Page 9 of 9 <br />ATTY/AGR.2017.212/Fresh Lifelines for Youth HSFA Funding Agreement FY 17-18 <br />
