<br />ORIGINAL <br /> <br />(08/26/96) <br /> <br />AGREEMENT <br /> <br />Crisis Intervention and Suicide Prevention <br /> <br />~EEMENT' made and entered into this ,~.¿ ~ day of <br />, 1996, by and between the CITY OF REDWOOD CITY, a municipal <br />corporation of the State of California ("City") and Crisis Intervention and Suicide Prevention, a <br /> <br />California nonprofit corporation ("Contractorll); <br /> <br />WITNESSETH: <br /> <br />WHEREAS, Contractor provides assistance to people who are trying to deal with their <br /> <br />personal, emotional and mental problems; and <br /> <br />WHEREAS, Contractor has requested financial assistance in order to conduct the program <br /> <br />for residents of City during the fiscal year commencing July 1, 1996, and ending June 30, <br /> <br />1997; and <br /> <br />WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum of <br /> <br />Five Thousand and No/100 Dollars ($5,000); <br />NOW, THEREFORE, the parties hereto do hereby agree as follows: <br /> <br />1 . <br /> <br />FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of Five Thousand <br /> <br />and No/100 Dollars ($5,000) for exclusive use by Contractor during the fiscal year <br />commencing July 1, 1996, and ending June 30, 1997, solely for the purposes described in <br /> <br />paragraph 2. <br /> <br />2. <br /> <br />USE OF FUNDS. Contractor shall use the funds provided pursuant to paragraph 1 <br /> <br />solely for the Program purposes described in Exhibit "All (Contract Objectives) attached hereto <br /> <br />and by this reference incorporated herein. <br /> <br />3. <br /> <br />CHANGES TO PROGRAM. No changes in the Program described in this agreement <br /> <br />which are funded by the financial assistance provided under paragraph 1 shall be made without <br /> <br />the prior written consent of City. <br /> <br />r - <br />
