<br />DES:dft <br /> <br />fl/20/91 (020/46) <br /> <br />performance, <br /> <br />or <br /> <br />failure <br /> <br />to <br /> <br />perform, <br /> <br />services <br /> <br />under <br /> <br />this <br /> <br />agreement. <br /> <br />9. <br /> <br />INSURANCE. <br /> <br />Consultant <br /> <br />shall <br /> <br />acquire <br /> <br />and <br /> <br />maintain <br /> <br />during <br /> <br />its <br /> <br />performance <br /> <br />of <br /> <br />services <br /> <br />hereunder <br /> <br />Workers' <br /> <br />Compensation insurance, employer's liability insurance coverage, <br />comprehensive general liability insurance coverage, and <br />professional liability insurance coverage, relating to its <br /> <br />services to be performed hereunder. <br /> <br />The amount of Workers' <br /> <br />Compensation insurance shall not be <br /> <br />less <br /> <br />than <br /> <br />the <br /> <br />statutory <br /> <br />minimum. <br /> <br />Employer's <br /> <br />liability, <br /> <br />comprehensive <br /> <br />general <br /> <br />liability, <br /> <br />and <br /> <br />professional <br /> <br />liability <br /> <br />(errors <br /> <br />and <br /> <br />omissions) <br /> <br />coverage <br /> <br />shall <br /> <br />be <br /> <br />not <br /> <br />less <br /> <br />than <br /> <br />Five <br /> <br />Hundred Thousand Dollars <br /> <br />($500,000.00) <br /> <br />combined single limit per <br /> <br />occurrence/accident <br /> <br />(as appropriate); provided that City may, in <br /> <br />its sole discretion, <br /> <br />approve such different amounts of coverage <br /> <br />as Consultant may propose, provided, <br /> <br />further, <br /> <br />that such amounts <br /> <br />as may be acceptable to City shall be reasonably related to the <br /> <br />risk <br /> <br />insured. <br /> <br />The <br /> <br />comprehensive <br /> <br />general <br /> <br />liability <br /> <br />insurance <br /> <br />shall include City, <br /> <br />its Council, <br /> <br />boards, <br /> <br />Commissions, <br /> <br />officers, <br /> <br />employees <br /> <br />and <br /> <br />agents <br /> <br />as <br /> <br />additional <br /> <br />insureds, <br /> <br />or <br /> <br />otherwise <br /> <br />evidence extension of coverage to said additional insureds. <br /> <br />Concurrently <br /> <br />with <br /> <br />the <br /> <br />execution <br /> <br />of <br /> <br />this <br /> <br />agreement, <br /> <br />Consultant shall furnish City with a certificate or certificates, <br /> <br />evidencing <br /> <br />issuance <br /> <br />of <br /> <br />all <br /> <br />insurance <br /> <br />required <br /> <br />hereunder, <br /> <br />and <br /> <br />copies <br /> <br />of <br /> <br />the <br /> <br />information <br /> <br />or <br /> <br />declaration <br /> <br />page(s) , <br /> <br />which <br /> <br />certificate(s) <br /> <br />and <br /> <br />information or declaration page (s) <br /> <br />shall be <br /> <br />4 <br />
