EXHIBIT "A" <br />SCOPE OF SERVICES <br />Funding for this agreement has been granted from the San Mateo County Health <br />System, Department of Behavioral Health and Recovery Services to support the <br />Redwood City and North Fair Oaks Alcohol and Other Drug Prevention Partnership. <br />StarVista will work in collaboration with the AOD Abuse Prevention Partnership to build <br />program capacity, and to increase the number of youth involved in positive activities <br />and supportive youth/adult relationships. <br />Specific tasks to be completed include: <br />StarVista staff will collaborate with the Sequoia Youth Advisory Board (YAB) to <br />implement at least one and up to two youth focus groups as part of community <br />assessment activities. <br />• StarVista will facilitate a Leadership and Resiliency program in the 2017/2018 <br />school year at Sequoia High School, recruiting at least 14 high-risk youth at <br />Sequoia High School to participate in this program and implementing the <br />following 4 program components: <br />weekly resiliency group meetings, including expressive art approaches, <br />psycho- education on drugs and alcohol, as well as individual skill - <br />building; <br />o community service activities; <br />o peer education; and <br />o outdoor activities. <br />• StarVista staff will attend monthly Prevention Partnership Steering Committee <br />meetings. <br />StarVista staff will attend 8 Prevention Training Academy sessions, hosted by <br />Behavioral Health and Recovery Services of San Mateo County. <br />Ensure appropriate County reporting and data collection requirements are met <br />including but not limited to entering and maintaining monthly PPSDS records. <br />StarVista will plan and facilitate a youth -led community hallmark activity which <br />includes the presentation of a youth Alcohol and Other Drug prevention project <br />to the broader community. The hallmark activity will reach a minimum of 100 <br />community members. <br />• StarVista staff will submit monthly documentation of Leadership and Resiliency <br />and Youth Advisory Board programs, including but not limited to agendas and <br />attendance sheets, to Redwood City 2020. <br />• Complete other tasks related the Redwood City 2020 AOD Prevention <br />REV: 11-09-17 RL <br />Page 10 of 11 <br />ATTY/AGR.2017.266/AOD StarVista <br />
