ACCEPTANCE OF PROPOSAL <br />Agent for Allied Van Line Cal T #145636 <br />Note: <br />For acceptance of this proposal for your move, please acknowledge receipt by sending <br />signed acceptance form enclosed to me via e-mail. This will ensure prompt response to <br />your needs in regards to your move, and confirm acceptance of our proposal and terms. <br />Price based on the following: <br />- Exclusive use of elevators and loading areas at origin & destination <br />- No interruptions due to construction <br />- Placement of furniture once only <br />- Relocation based on inventory provided at time of survey & number of cartons <br />indicated <br />- Based on relocation plan <br />- Customer must be present at all times during move <br />- Label all items to be moved with the destination room number or office <br />- Post signage at the new space designating room or office numbers <br />- For cubicles, main power and cabling disconnect / re -connect must be done by <br />customer; cost of obtaining permits not included <br />- There is a 4 hour minimum on weekdays and a 6 hour minimum on weekends <br />The costs listed above are estimates.
