EXHIBIT "A" SCOPE OF SERVICES Funding for this agreement has been granted from the San Mateo County Health System, Department of Behavioral Health and Recovery Services to support the Redwood City and North Fair Oaks Alcohol and Other Drug Prevention Partnership. Peninsula Conflict Resolution Center ('PCRC) will work in collaboration with the AOD Prevention Partnership to increase the number of community members with knowledge and skills for AOD prevention. Specific tasks will include: • PCRC staff will attend Prevention Partnership Steering Committee meetings when the topics apply to their scope of work and/or when staff has availability. • Ensure appropriate county reporting and data collection requirements are met, including but not limited to Primary Prevention Substance Abuse Disorder Service (PPSDS) data system entries. • PCRC staff will attend 3 Prevention Training Academy sessions, hosted by Behavioral Health and Recovery Services of San Mateo County. • PCRC staff will plan, conduct outreach, and facilitate 3 prevention -focused, evidence -based community education trainings, informed by Prevention Training Academy classes. PCRC will ensure: o Trainings are conducted for families, community members, and/or service providers o Trainings have a minimum of 12 participants in attendance Complete other tasks related to the Redwood City 2020 AOD Prevention Partnership as assigned and agreed upon by PCRC and Redwood City 2020. Consultant will invoice Redwood City 2020 twice per fiscal year, providing documentation of actions. If all deliverables are met, payment will be two equal installments amounting to the full and total compensation unless otherwise determined pursuant to actions invoiced. Unless earlier terminated, the term of this Agreement will be effective from July 1, 2017 until June 30, 2018.
