Occupancy Reports: Annually, on June 30th of every year of the Affordability <br />Period, Owner shall provide to the City a report containing, at a minimum, the following <br />information: <br /> <br />i) Total number of Assisted Units that are occupied; <br />ii) Total number of Assisted Units that are vacant; <br />iii) A certified rent roll for Assisted Units containing the unit numbers, <br />the lease commencement and expiration dates, the numbers of bedrooms <br />and bathrooms within the rented Assisted Units and the rental rate for <br />each rented Assisted Unit; <br />iv) For each Assisted Unit, the unit size, household size, household <br />income, income limits for a qualifying lessee, maximum affordable rents <br />for a qualifying lessee, utility allowance, and total monthly rent; <br />v) Evidence that the income for each tenant of an Assisted Unit does <br />not exceed the maximum income for a qualifying lessee; <br />vi) The total number of units that are occupied by staff, if any; <br />vii) Such other information as may be reasonably required by City to <br />determine compliance with the terms of this Agreement. <br /> <br />IN WITNESS WHEREOF, Owner has executed this Declaration on the date first <br />written above. <br /> <br /> “Owner” <br /> <br />MP Mosaic Garden Associates, L.P. <br />a California limited partnership <br /> <br />By: MP Atherton Court, LLC, <br />a California limited liability company, <br />Its general partner <br /> <br />By: Mid-Peninsula Seven Trees, Inc. <br />A California nonprofit public benefit <br />corporation <br />Its sole member and manager <br /> <br />By:________________________ <br />Name: Matthew O.
