<br />EXHIBIT A <br /> <br />CITY OF REDWOOD CITY <br />YOUTH AND FAMILY ENRICHMENT SERVICES <br />CRISIS INTERVENTION AND SUICIDE PREVENTION CENTER <br />FISCAL YEAR 2005/2006 <br /> <br />CONTRACT OBJECTIVES <br /> <br />Quantifiable Goals to be Provided With Requested Performance Indicators <br />Funds <br />Provide crisis intervention and suicide prevention CISP staff and volunteers maintain a database <br />services to 1,500 individuals via the 24-hour crisis lines tracking system and record each call, noting <br />to Redwood City residents length of call, primary reason for call and <br /> assessment of Dotential suicide risk. <br />Youth Intervention Team will conduct one-hour Attendance tracked at presentations by <br />educational presentations on suicide and crisis anonymous demographic information sheet <br />intervention and prevention for 250 youth in Redwood which also asks for participant's city of <br />City residence. <br />Conduct 8 crisis interventions in Redwood City CISPC staff report on student demographic <br /> information, number of contacts, outcome of <br /> the intervention. <br />Demonstrate student knowledge of suicide prevention Students who attend suicide prevention <br />and intervention techniques as evidence by average presentation will be administered a <br />scores above 70% on Dresentation Dost test. Dresentation Dost test. <br />Train and develop 5 Crisis Center volunteers from 50-hr volunteer trainings series (mandatory for <br />Redwood City all volunteers), participants complete a <br /> demoqraphic questionnaire. <br /> <br /># of Unduplicated RWC Residents/Households to be Assisted (Only programs serving individuals <br />may report individuals.
