<br />1 E. Delivery of Financial Statements, Auditors' Reports. <br />2 Within 120 days after the close of each Fiscal Year, Contractor shall deliver to the <br />3 Agency four (4) copies of the audited consolidated financial statements of the <br />4 operations pursuant to this Agreement for the preceding Fiscal Year. These statements <br />~- 5 shall have been examined by and shall be accompanied by the report of an independent <br />6 certified public accountant containing such accountant's representation that it has <br />7 examined the Contractor's financial statements in accordance with Generally Accepted <br />8 Auditing Standards (GAAS) and the accountant's opinion that such statements have <br />9 been prepared in accordance with Generally Accepted Accounting Principles (GAAP) <br />10 consistently applicable and fairly reflect the results of operations and Contractor's <br />11 financial condition. <br />12 <br />13 8.02.3 Solid Waste Records. Records shall be maintained by Contractor <br />14 for Agency relating to: <br />15 <br />16 A. Service recipient services and billing; <br />17 <br />18 B. Character, weight and volume of Solid Waste by type (e.g., <br />19 Solid Waste, Recyclable Materials, and Plant Materials), especially as related to <br />20 reducing and diverting Solid Waste. Where possible, information is to be separated <br />21 between service recipients residing in Single-Family and Multi-Family Dwelling Units <br />22 from occupants of Commercial, industrial and institutional I'remises; <br />23 <br />24 C. Routes; <br />25 <br />26 D. Facilities, equipment and personnel used; <br />27 <br />28 E. Facilities and equipment operations, maintenance and <br />29 repaIr; <br />30 <br />31 F. Processing and Disposal of Solid Waste; <br />32 <br />33 G. Complaints; and, <br />34 <br />35 H. Missed pick-ups. <br />36 <br />37 Contractor shall maintain records of all Solid Waste Collected in <br />38 the Agency for the period of this Agreement and all extensions to this Agreement or <br />39 successor Agreements. In the event Agency requests certain records or Contractor <br />40 discontinues providing services to Agency, Contractor shall provide all records of all <br />41 Solid Waste requested to Agency within thirty (30) days of discontinuing service. <br />42 Records shall be in chronological and organized form and readily and easily <br />43 interpreted. <br />44 <br /> Atty j Agrj2005.022 <br /> 060205 -41- <br />
