<br />6. 4/1- /3 <br /> <br />EXHIBIT A <br />EMERGENCY WORK - 5TH AVENUE PUMP STATION <br /> <br />SCOPE OF WORK <br /> <br />All labor, materials, equipment and other effort in replacing the damaged pipes with a <br />reinforced concrete box culvert within the City's storm drainage easement for the 5th <br />Avenue Pump Station. Work shall include constructing new reinforced concrete box <br />culvert (RCBC) as shown on the attached drawing Exhibit B, removal and replacement <br />of concrete pads for adjacent mobile homes, including utility relocation, pavement <br />restoration, temporary bypass, and protection of the easement canal. <br /> <br />Prior to construction, the City-hired Designer shall submit to the City for approval a <br />Basis of Design Report and design including topographic survey for the RCBC. The <br />Basis of Design report and design shall include but not limited to, the size of the RCBC <br />based on design flows and existing conditions; design grades for the RCBC; type of <br />RCBC proposed; relocation of City's water, sewer, storm and street lights; restoration of <br />all surface improvements; traffic control plan; and coordination with other private utility <br />companies. <br /> <br />The design may be completed after the Notice to Proceed for construction has been <br />issued. The Contractor shall conduct demolition and removal of storm drain pipe, and <br />site preparation anticipating the recommended design alternative. <br /> <br />Record Drawings of the completed RCBC and other improvements shall be submitted <br />within 30 calendar days after acceptance of the project. <br /> <br />Construction Cost: The not to exceed price for constructing the culvert including all <br />permits, contingencies, and fees is Seven Hundred Twenty Thousand and No/100 <br />Dollars ($720,000.00). <br /> <br />Schedule: The construction of the culvert shall start on or about September 15, 2005, <br />and be completed by approximately November 15, 2005, unless extended mutually by <br />both parties. <br /> <br />sllibrary/agreementslaward/Emergency Work - 5h Avenue Pump Station <br /> <br />Atty/Agr/2005.064 <br />083105 <br /> <br />9 <br />
