<br />Agreement with Kainos Home & Training Center <br />for the Developmentally Disabled <br /> <br />Funding for Property Acquisition <br /> <br />Exhibit A <br /> <br />PROGRAM SERVICES <br /> <br />A. <br /> <br />ACTIVITIES <br /> <br />1. <br /> <br />Assianment and Transfer of Aareement <br /> <br />Notwithstanding Paragraph 7 herein, this Agreement may be assigned by <br />Contractor to and be assumed by a very low-income household (hereinafter <br />called "Beneficiary") provided that very low income "Beneficiary" shall assume all <br />rights and obligations of the Contractor hereunder, in form and content <br />satisfactory to the City. <br /> <br />2. <br /> <br />Acquisition <br /> <br />With funds provided under this Agreement, and funds to be deposited by <br />Contractor hereunder, Contractor shall acquire property ("Project') at 2033 <br />Jefferson Avenue, Redwood City. Contractor shall deposit sufficient fees to <br />acquire the property plus funds for non recurring escrow fees in order to <br />complete this sale at a purchase price of ~' . Upon completion of this <br />acquisition, Contractor shall provide independent .ving for 6 developmentally <br />disabled adults on the subject property. <br /> <br />3. ~1\~ ~ <br />Marketina 11/ <br /> <br />Contractor shall select occupants from a pool of applicants residing in the <br />incorporated limits of the City of Redwood City. Contractor shall include as part of <br />its marketing efforts, outreach to developmentally disabled persons who are <br />homeless and at-risk of being homeless households transitioning through one of <br />the various homelessness assistance programs operating in the City of Redwood <br />City. Prospective applicants must show willingness to pay and ability to partner <br />in the independent living environment provided in the dwelling to be purchased <br />under this contract. Preference will be given to eligible households who are <br />qualified to live independently and have transitioned from a Kainos residential <br />group facility (or applicants transitioning from similar residential group facilities) <br />and are receiving ongoing supportive and vocational services from a qualified <br />provider authorized to do so by the Golden Gate Regional Center. <br /> <br />1 <br />
