<br />bingo game without first obtaining a permit therefor as herein <br /> <br />provided. <br /> <br />An application for a bingo game permit shall be made <br /> <br />to the Chief of Police on a form provided, or in the manner <br /> <br />prescribed, by the Chief of Police. <br /> <br />The application shall <br /> <br />contain the following information: <br /> <br />"(a) <br /> <br />The name and address of the applicant organization; <br /> <br />"(b) <br /> <br />The name and address of the person(s) authorized or <br />required by the applicant organization to sign <br />contracts, legal documents, or otherwise to bind <br />or commit the organization, or act for on on behalf <br />of the organization; and <br /> <br />"(c) <br /> <br />A brief description of the nature or purpose of <br />the applicant organization. <br /> <br />"The application shall be signed by the person or persons <br /> <br />designated in (b) above. <br /> <br />"Upon submission of the application as in this section <br /> <br />required, the certifications specified in Section 8A.5, and <br /> <br />the payment of the fee specified in Section 8A.6, a permit <br /> <br />shall be issued, effective for a period of one year from the <br /> <br />date of issuance. <br /> <br />A permit may be renewed, or a new permit <br /> <br />issued, upon compliance with the requirements of this chapter <br /> <br />pertaining to the issuance of an initial permit. <br /> <br />"Section 8A.5 <br /> <br />Evidence of Tax Exemption Status to <br /> <br />Accompany Permit Application. <br /> <br />Any organization applying for <br /> <br />a bingo permit shall submit with its application therefor, a <br /> <br />certificate from the Franchise Tax Board of tax exempt status <br /> <br />under California Revenue and Taxation Code Section 23701(d), <br /> <br />and a certification from the U. S. Department of Internal <br /> <br />Revenue that a contribution or gift to the applicant would be <br /> <br />a charitable contribution under Section 170(c) (2) of the U. S. <br /> <br />Internal Revenue Code of 1954 as amended. <br /> <br />"Section 8A.6 <br /> <br />Permit Fees. <br /> <br />The permit fee for each permit <br /> <br />issued, renewed, or reissued pursuant to the provisions of this <br /> <br />-2- <br />
