Unpaid accounts exceeding 30 days <br />after the original invoice date without prior approval will be charged <br />1.5% per month late payment SERVICE CHARGE. <br /> <br />Note: Rates subject to change annually in January. <br /> <br />Page I of I <br /> <br />
