6.2.A. - Page 14 Name: Home Address: Agreement to Hold Harmless the City of Redwood City ID Number: State: Telephone Number: Name and description of event planned: Stanford "Treeathlon" Date of Activity: March 3 1 & 4th, 2018 I hereby waive, release and discharge any and all claims for damages for death, personal injury or property damage which I may have, or which hereafter accrue to me, against the City of Redwood City, the City Council, their officers, officials, employees and volunteers, any other involved municipalities or public agencies (collectively "Entities") as a result of my participation in the event. This release is intended to discharge the Entities from and against any and all liability arising out of or connected in any way with my participation in the event, even though that liability may arise out of the negligence or carelessness on the part of persons or Entities mentioned above. I further understand that accidents and injuries can arise out of the event, including, but not limited to, over-exertion, running, riding bicycles on surfaces not intended for that purpose or swimming in the San Francisco Bay. Knowing the risks, nevertheless, I hereby agree to assume those risks and to release and to hold harmless all of the Entities mentioned above who (through negligence or carelessness) might otherwise be liable to me (or my heirs or assigns) for damages.
