EXHIBIT "A" <br />SCOPE OF SERVICES <br />Funding for this agreement has been granted from the San Mateo County Health System, <br />Department of Behavioral Health and Recovery Services to support the Redwood City and <br />North Fair Oaks Alcohol and Other Drug Prevention Partnership. <br />Friends for Youth ("FFY") will work in collaboration with the ACD Abuse Prevention <br />Partnership to build program capacity in increasing assets and resilience of vulnerable <br />populations through youth development, to increase the number of youth and adults in <br />supportive and caring relationships, and to provide youth development training and support <br />for adult mentors in the community. <br />Specific tasks will include: <br />FFY will provide at least 5 prevention -focused, evidence -based trainings to at least 15 <br />new mentors to work with at -risk youth from Redwood City and North Fair Oaks. <br />Trainings will highlight the importance of positive familial & other relationships and <br />encourages pro -social behavior development in mentees. <br />FFY Program Coordinators will provide regular contact (min. 75 contacts) with <br />Mentors of at -risk youth from Redwood City and North Fair Oaks to provide <br />prevention -focused, evidence -based support. <br />FFY staff will continue to improve internal evaluation and program improvement <br />practices in support of mentors and mentees as they relate to AOD prevention efforts. <br />• FFY staff will attend monthly Prevention Partnership Steering Committee meetings. <br />FFY staff will attend 8 Prevention Training Academy sessions, hosted by Behavioral <br />Health and Recovery Services of San Mateo County. <br />Ensure appropriate County reporting and data collection requirements are met <br />including but not limited to entering and maintaining monthly PPSDS records. <br />Submit monthly documentation of mentor trainings and coaching, Including but not <br />limited to agendas and attendance sheets, to Redwood City 2020. <br />Complete other tasks related the Redwood City 2020 ACD Prevention Partnership as <br />assigned and agreed upon by Friends for Youth and Redwood City 2020. <br />Consultant will invoice RWC 2020 twice per fiscal year, providing documentation of actions <br />taken during that proceeding invoice period. If all deliverables are met, payment will be in two <br />equal installments pursuant to invoices amounting to the full compensation. <br />The terms of this agreement will be effective from July 1, 2017 until June 30, 2018. <br />REV: 11-09-17 RL <br />Page 10 of 10 <br />A7TY/AGR.2017.264/AOD - Friends for Youth <br />
