All rights reserved. <br /> <br />City of Redwood City: Overflow Emergency Response Plan <br />A-2a <br />Regulatory Notifications Packet <br />Category 1 SSO Reporting Checklist <br />Use this Checklist for Category 1 SSOs only <br /> <br />STEP 1: Receive call from crew. <br /> <br /> <br /> STEP 2: 2-hour Notification <br />If the SSO is greater than or equal to 1,000 gallons, notify CalOES within 2 hours of the time the agency was <br />notified of the SSO. <br /> <br /> Notify CalOES at (800) 852-7550: <br /> <br />o Date Called: <br />o Time called: : ☐AM ☐PM <br />o CalOES Control number: <br />o City personnel who called CalOES: Name <br /> Title <br />o Individual they spoke to at CalOES: <br /> Notify San Mateo County Environmental Health Services <br /> Notify Utilities Specialist – Storm or Utilities Specialist – Sewers, if applicable. <br /> <br />STEP 3: Within 48 hours after awareness of SSO <br /> <br /> Only if 50,000 gallons or more was not recovered, implement Water Quality Monitoring Plan. <br /> <br />STEP 4: Within 3 Days after awareness of SSO <br /> <br /> Submit a Draft Spill Report using the CIWQS online reporting database. <br /> <br />STEP 5: Within 15 Days after response conclusion <br /> <br /> LRO must certify the Spill Report using the CIWQS online reporting database. Amendments to the Spill Report may <br />be made for up to 120 days following the conclusion of the SSO Response. <br /> <br />STEP 6: Within 45 Days after SSO end date <br /> <br /> Within 45 days after the SSO end date, submit an SSO Technical Report using the CIWQS online reporting <br />database only if 50,000 gallons or more was spilled to surface waters. <br /> <br /> <br />This form completed by: <br /> Name Title Date <br />6.2.A. - Page 127
