EXHIBIT "A" <br />SCOPE OF SERVICES AND FEE <br />Expand the survey of the original Professional Services Agreement dated April 9h, 2018, to encompass <br />additional areas within Red Morton Park to support the design of the Veterans Memorial Senior <br />Center and to provide a survey of intersections adjacent to the project to inform the design of <br />traffic calming measures. <br />PART A—ONSITE SURVEY EXPANSION <br />$7,850 <br />• Expand the previously prepared survey to encompass the areas shown on the attached <br />proposal from Sandis (Exhibit "B"). The information will be integrated to the prior survey and <br />reissued. <br />PART B — INTERSECTION SURVEYS <br />$26,520 <br />Provide topographic and boundary surveys for each of the intersections shown on the <br />attached proposal from Sandis (Exhibit "C"). The survey is to inform the design of traffic <br />calming measures and will generally include the following: <br />• The survey will extend from edge of right of way to edge of right of way. <br />• The survey will extend down each leg of the intersection for 150 feet past the curb <br />return. For midblock locations 300 linear feet of the street will be surveyed. <br />• All visible surface features will be shown. <br />• Underground utilities will be shown based on visible surface features, measured depth of <br />storm and sewer lines at structures, and available record documents. Mechanical utility <br />locating is specifically excluded. Detailed as -built survey of traffic signal equipment and <br />wiring (where present) is excluded. <br />• In addition to the topographic survey, the location of the public right of way will be shown for <br />each intersection based upon available public records. <br />The survey will be delivered in pdf and AutoCAD format. The AutoCAD file will include a <br />Civil3D surface reflecting the existing ground surface.
