EXHIBIT A <br />1. Scone of Work <br />Contractor will provide interior cleaning and inspection services for three potable water tanks in <br />Redwood City, California: <br />• Wilmington Reservoir; <br />• Peninsula Reservoir #1; and <br />• Carson Reservoir. <br />For each tank, Contractor will: <br />1. Coordinate with City staff to select appropriate day(s) for tank diving and cleaning. <br />2. Mobilize to and from the project site. <br />3. Set-up and breakdown equipment. <br />4. Prepare for dive. <br />5. De -chlorinate all discharge water. <br />6. Remove up to three inches of sediment from the floor of each tank. <br />7. Time remaining in the second 8 -hour day in the Carson tank will be used if needed to <br />clean areas of the sloped walls where sediment may have accumulated. <br />8. Visually inspect the interior and exterior of each tank. <br />9. Provide written reports, with photos, for each tank summarizing the results of the visual <br />inspections and sediment removal activities. <br />10. Provide a DVD of each dive to City staff. <br />Additional Work: <br />Upon request by the City, Contractor will remove any remaining sediment from the sloping walls <br />of Carson Reservoir and/or more than 3 inches of sediment from tank floors. <br />2. Pavment Terms <br />The total not to exceed amount is $25,176. Contractor will receive a flat rate of $13,792 for all <br />work outlined herein, excluding Additional Work, on the three potable water tanks identified <br />below. Additional charges for sediment bags, overtime (i.e. cleaning and inspection that exceeds <br />two eight-hour workdays), removal of more than 3 inches of sediment from tank floors, and <br />cleaning of sloped walls at Carson Reservoir, if necessary and approved by the City, will be <br />charged at the rates indicated below and will be a separate line item on the invoice. <br />Description <br />Tank inspection and cleaning at Wilmington, Peninsula #1, and Carson <br />Reservoirs <br />Additional Work (sediment removal from sloped walls of Carson <br />Reservoir or more than 3 inches of sediment from flat floor surfaces) <br />Overtime — 2 hours allowed per day <br />Day rate — 8 hours or any part thereof <br />Sediment Bags <br />TOTAL NOT TO EXCEED <br />Page 6 of 6 <br />ATN/AGR/2018.072/INLAND POTABLE SERVICES, INC.
