EXHIBIT "A" SCOPE OF SERVICES Funding for this agreement has been granted from the San Mateo County Health System, Department of Behavioral Health and Recovery Services to support the Redwood City and North Fair Oaks Alcohol and Other Drug Prevention Partnership. John W. Gardner Center for Youth and Their Communities at Stanford University will continue its research and evaluation role in the Redwood City/North Fair Oaks ACD Abuse Prevention Partnership. Key activities will include: • Contribute a research -oriented perspective to the AOD Prevention Partnership steering committee meetings and in consultation with Prevention Partnership partners. • Gardner Center staff will attend 5 Prevention Training Academy sessions and support ACD Prevention Coordinator to incorporate data and data -informed strategies into Prevention Partnership steering committee meetings and strategic planning. • Gardner Center staff will help Prevention Partnership members define priorities and measureable objectives for AOD Prevention Partnership work plans for FY 2018-19 and 2019-20 through training, facilitated discussion, consultation, and/or development of materials. • Ensure appropriate County reporting and data collection requirements are met Including but not limited to submitting monthly Prevention Substance Use Disorder Data Service (PPSDS) data system records. • Gardner Center staff will attend monthly Prevention Partnership Steering Committee meetings, • Complete other tasks related the Redwood City 2020 AOD Prevention Partnership as assigned and agreed upon by John W. Gardner Center for Youth and Their Communities at Stanford University and Redwood City 2020. Consultant will invoice Redwood City 2020 twice per fiscal year, providing documentation of actions. if all defiverables are met, payment will be two equal installments amounting to the full and total compensation unless otherwise determined pursuant to actions invoiced.
