3409 <br />2. <br />Dates, times, and names of meetings or events attended. <br />3410 <br />3. <br />Dates, times, and names of school(s) where presentations were performed. <br />3411 <br />4. <br />Completion of other activities specified in the then -current Three -Year Public <br />3412 <br />3431 <br />Education Plan. <br />3413 <br />5. <br />Other educational activities as may be requested by Agency. <br />3414 C. Determination and Payment of Liquidated Damages. In accordance with the <br />3415 requirements of Section 14.07, Contractor shall provide a report that identifies any <br />3416 non-compliance with performance measures listed in Attachment J and include <br />3417 calculation of the Liquidated Damages due. Contractor may include with its report a <br />3418 written request to meet with Agency's Manager or his or her designee to discuss <br />3419 Contractor's evidence refuting the basis for assessing Liquidated Damages <br />3420 pertaining to unacceptable employee behavior. In such cases, Contractor shall <br />3421 include with its report evidence in writing and written testimony of its employees and <br />3422 others relevant to the incident(s)/non-performance. Agency's Manager or his or her <br />3423 designee will provide Contractor with a written explanation of his or her <br />3424 determination on each incident(s)/non-performance. The decision of Agency's <br />3425 Manager or his or her designee shall be final. <br />3426 <br />D. Account Summary. Provide the following account summary information in table <br />3427 <br />format: <br />3428 <br />1. Number of Customers in each Rate and Charge category. <br />3429 <br />2. Total number of Residential, Commercial, and Drop Box Customers <br />3430 <br />subscribing to Solid Waste, Targeted Recyclable Materials, and Organic <br />3431 <br />Materials Collection service listed separately by Service Sector and material <br />3432 <br />type. <br />3433 <br />3. Percentage of Customers subscribing to Targeted Recyclable Materials <br />3434 <br />Collection service (listed separately for Multi -Family, Commercial, and Drop <br />3435 <br />Box Customers), which shall be equal to the total number of Targeted <br />3436 <br />Recyclable Materials Customers divided by the total number of Solid Waste <br />3437 <br />Customers. <br />3438 <br />4. Percentage of Customers subscribing to Organic Materials Collection service <br />3439 <br />(listed separately for Multi -Family, Commercial, and Drop Box Customers), <br />3440 <br />which shall be equal to the total number of Organic Materials Customers <br />3441 <br />divided by the total number of Solid Waste Customers. <br />3442 <br />5. Weekly gallons or cubic yards of service provided to Residential, Commercial, <br />3443 <br />and Drop Box Customers subscribing to Solid Waste, Targeted Recyclable <br />3444 <br />Materials, and Organic Materials Collection service listed separately by Service <br />3445 <br />Sector and material type for one week. <br />3446 <br />E. Operational Data. Contractor shall submit the following: <br />3447 <br />1. A summary of Collection route operational data including: average number of <br />3448 <br />Customers and Containers serviced per route per Day for each Collection <br />3449 <br />route; average number of actual both on -route and off -route hours per Day by <br />3450 <br />route (distinguishing between Standard Collection Vehicle Routes and Special <br />3451 <br />Collection Vehicle Routes, if appropriate); <br />REV: 03-16-18 MI <br />Page 83 of 126 <br />ATTY/AGR.2018.054/Recology of San Mateo <br />
