ORIGINAI <br />AMENDMENT NO. 4 TO EXTEND AND AMEND AGREEMENT FOR <br />PROFESSIONAL SERVICES <br />LESLIE PARKS <br />This Amendment No. 4 is entered into and effective as of the 2S day of <br />(d , 2019, extending and amending the agreement dated June <br />11, 2018 the "Agreement") by and between the City of Redwood City, a charter city and <br />municipal corporation of the State of California ("City"), and Leslie Parks, ("Consultant") <br />(collectively, the "Parties") <br />RECITALS <br />A. On June 11, 2018 the Parties executed Agreement for Professional <br />Services ("Original Agreement); and <br />B. On November 8, 2018, the Parties executed Amendment No. 1 to extend <br />and amend Agreement for Professional Services ("Amendment No. 1"); and <br />C. On April 1, 2019, the Parties executed Amendment No. 2 to extend the <br />Original Agreement ("Amendment No. 2"), and <br />D. Effective June 1, 2019, the Parties executed Amendment No. 3 to extend <br />and amend Agreement for Professional Services ("Amendment No. 3"; the Original <br />Agreement, Amendment No. 1, Amendment No. 2 and Amendment No. 3 are <br />collectively referred to herein as the "Agreement"); and <br />E. The Parties have negotiated and agreed to amendments attached to and <br />incorporated in by reference as Exhibit "A," Scope of Services and Fee. <br />NOW, THEREFORE, in consideration of these recitals and the mutual covenants <br />contained herein, the Parties agree as follows: <br />1. In addition to those services contained in the Agreement, Consultant will <br />provide those services described in Exhibit "A." <br />2. City will pay Consultant for all work associated with those services <br />described in Exhibit "A" on a time and materials basis not -to -exceed Fifty Two Thousand <br />Three Hundred Twenty Five Dollars ($52,325), for an aggregate amount 'of Two <br />Hundred Twenty Eight Thousand Three Hundred Twenty Five Dollars ($228,325). <br />Consultant will provide City, on a monthly basis, copies of invoices sufficiently detailed <br />to include hours performed, hourly rates, and related activities and costs for approval by <br />City. <br />3.
