List and describe services and facilities that meet the needs of homeless persons, particularly chronically homeless individuals and families, families with children, veterans and their families, and unaccompanied youth. If the services and facilities are listed on screen SP-40 Institutional Delivery Structure or screen MA-35 Special Needs Facilities and Services, describe how these facilities and services specifically address the needs of these populations. The County homeless system includes an array of prevention programs, providing financial assistance to households at-risk of losing their housing. Most homelessness prevention work is conducted by the Core Service Agencies. The Supportive Services for Veteran Families program and the Next Step Veterans Center focus in particular on preventing and ending homelessness for veterans. The Human Services Agency also funds a new Diversion program that is part of the community’s Coordinated Entry System. Diversion is problem solving to identify a solution to a household’s housing crisis and prevent entry into emergency shelter or a homeless program. The overarching goal of diversion is to prevent entry into homelessness wherever possible. Diversion helps people experiencing a housing crisis maintain housing and avoid entering the homeless system – and helps ensure that scarce resources, especially the most intensive housing supports, can be dedicated to serve households with the greatest need. Over the last decade, San Mateo County has partnered with cities and non-profit providers to implement multi-disciplinary Homeless Outreach Teams (HOT), focusing on areas with a large number of unsheltered and chronically homeless adults (including downtown San Mateo, Redwood City, East Palo Alto, South San Francisco and parts of the Coastside). HOT teams conduct outreach, engage with unsheltered homeless people, work to identify their individual service needs, then develop and execute person-centered housing plans. The HOT teams are multidisciplinary and include key partners such as law enforcement, behavioral health staff from the Health System, Core Service Agencies, and other partners. Interim Housing is a critical element of the County’s homeless crisis response system; it is a short-term intervention designed to act as a safety net for households who are unsheltered or who are in the midst of housing crisis and have no housing options. These programs provide an entry point into stabilization services and help move households towards permanent housing quickly. Individuals and families who enter interim housing receive immediate short-term case management to address and resolve current crises, with a focus on addressing barriers that prevent households from re-entering housing. Rapid re-housing provides homeless individuals and families with a short term rental subsidy. Services include help with locating housing, as well as time-limited case management focused on maintaining stability in housing. RRH is one of the most cost effective interventions for people experiencing homelessness and is a critical element of the community’s homeless crisis response system.
