<br /> Consolidated Plan REDWOOD CITY 102 <br />OMB Control No: 2506-0117 (exp. 06/30/2018) <br />Transportation Services. Services with the highest use include Medical Case Management, <br />Outpatient/Ambulatory Care, Medical Transportation Services, and Food Bank/Home-Delivered meals. <br />Describe the strengths and gaps of the service delivery system for special needs population <br />and persons experiencing homelessness, including, but not limited to, the services listed <br />above <br />San Mateo County’s homeless service delivery system benefits from a commitment to make progress <br />toward ending homelessness, a strong history and culture of inter-agency collaboration to improve <br />service delivery for homeless people, and several major funding streams that can be leveraged and <br />aligned to support the development and refinement of an effective Homeless Crisis Response System. <br />Over the past several years, the community has put in place all the key elements of the system, <br />including outreach (HOT), Coordinated Entry, interim housing (emergency shelters and short-term <br />transitional housing), an expanding supply of rapid re-housing and permanent supportive <br />housing. Robust performance measurement and data analysis is used to assess progress and make <br />improvements on an ongoing basis. <br />System challenges include the need for greater targeting and prioritization of unsheltered individuals <br />and families, right-sizing the system to speed movement from homelessness to housing, and expanding <br />permanent housing options in a high cost rental market. While most families in the community <br />experiencing a housing crisis can access shelter or transitional housing, unsheltered homelessness <br />among single adults continues to be an ongoing challenge in San Mateo County. Single adults make up <br />the majority of homeless people in the community, and there is a need to invest proportionally greater <br />resources for single adults while maintaining the commitment to families and ending family <br />homelessness. There are also challenges related to program access in both family and adult programs <br />and a need for greater prioritization based on housing need and vulnerability. <br />Provide a summary of the strategy for overcoming gaps in the institutional structure and <br />service delivery system for carrying out a strategy to address priority needs <br />San Mateo County’s 2016-2020 Strategic Plan to End Homelessness outlines five goals for addressing <br />challenges in the homeless services delivery system: <br />1) Create a Housing Crisis Resolution System that includes a coordinated entry system, robust shelter <br />diversion program, right-sizing of interim housing programs, expansion of rapid re-housing capacity, and <br />maximization of permanent supportive housing programs. <br />2) Ending veteran homelessness by 2017 by creating and implementing an optimal structure that <br />includes a registry, prioritization of veterans for housing assistance program access, and ensuring case <br />management for homeless veterans is focused on housing. <br />6.3.F. - Page 114
