EXHIBIT A 1. Scope of Work Contractor will perform boiler removal and replacement and staff training services at Red Morton Community Center in accordance with the services described below within one year of the Effective Date. Facility Address Equipment Model Description Manufacturer Red Morton 1120 Roosevelt Camus DFNH0751 DynaFlame near- Community Center condensing hot water boiler Contractor will: • Disconnect old boiler piping, flue, gas, and electrical. • Roll the old boiler out of the boiler room through the lobby for disposal. • Roll the new boiler through the lobby into the boiler room and place on existing pad. • Run new piping to the new boiler from existing connections on the wall. • Tie into the same flue with a reducer and adapters. • Wire new boiler and primary pump. • Start up boiler and verify operation. • Increase building loop pressure to 30 psi for stable boiler operation. • Clean tip room of any tools, debris, and installation materials. • Train staff on operation of the unit. 2. Payment Terms Contractor shall provide the services outlined herein for a flat fee of $28,928. 13. This amount includes labor, expected parts, and equipment required to perform the boiler removal and replacement tasks outlined in Section 1 (Scope of Work). Fees will be billed upon completion of boiler replacement and staff training. Contractor shall submit an invoice which includes a description of the services performed.
