The documents you have submitted require the following items: <br /> <br />) Insured must be the same name as shown on the City's Agreement. <br /> <br />) Workers' Cornpensation: Statutory minimum. <br /> <br />) Commercial General Liability: $1,000,000 per occurrence and $1,000,000 aggregate for <br />bodily injury, personal injury and property damage. <br /> <br />) Automobile Liability: $1,000,000 combined single limit. <br /> <br />) General Liability must be "Occurrence" based. <br /> <br />) The limits of liability must be as stated in the Agreement. <br /> <br />) Professional Liability: $1,000,000 per claim and aggregate. <br /> <br />) 30-day notice of cancellation of insurance for any reason, including nonpayment of premium <br />or reduction of coverage. (Revise Notice of Cancellation) <br /> <br />) Provide insurance certificate as shown on Attachment "A". <br /> <br />) Naming City, its Council, commissions, boards, cornmittees, officers, ernployees, and agents <br />as additional insureds on the Insurance Certificate. State project name and work description <br />on the Insurance Certificate. <br /> <br />( <br /> <br />) Provide Primary Insurance endorsements. <br />(Attachments "B-1" & "B-2") <br /> <br />) Insurance cornpany affording coverage must be an admitted, "A"-rated company in the State <br />of California.
