3 <br />EXHIBIT "A" <br />SCOPE OF SERVICES AND FEE <br />FireFlex Yoga Intermediate Program <br />• Classes to be delivered from 0800-0900 at specified stations <br />• 4 classes to be delivered per month per shift, for a total of 12 classes per month <br />• Each class will last for 60 minutes <br />• $150 per a class <br />• Two instructors will be delivering classes at the same time <br />• Yoga equipment will be provided by instructor <br />• All work will be performed by Contractor or her designated representative <br />FireFlex Yoga Video Program (Extended Program) <br />• A set of 8 FireFlex Yoga videos accessible through the FireFlex Yoga website will be <br />made available to the department at no additional cost for the life of the agreement. <br />Payment Terms <br />• Monthly fee: One Thousand Nine Hundred Eighty Dollars ($1,980) for a total amount not <br />to exceed Twenty Three Thousand Seven Hundred Sixty Dollars ($23,760) for the year. <br />REV: 07.24-19 RL <br />ATTY/AGR.Amend No. 1/Active Wellness LLC <br />Page 3 of 3 <br />
