C. - Page 28 <br />Attachment B — Cost Proposal Schedule <br />Proposers shall provide a comprehensive cost proposal schedule in Chapter 5 — Proposal Cost. <br />Proposer should include: <br />• hourly labor rate by class, including straight time and overtime rates (if applicable), <br />• equipment rates by class, including straight time and overtime rates (if applicable), <br />• material markup, <br />• annual preventative maintenance cost (flat fee, should include all labor, equipment, <br />parts, and associated costs), <br />• estimated annual cost for parts and materials, <br />• estimated cost range for major repairs by repair type, <br />• rate for adding/deleting pumps subject to the agreement. <br />All related costs to complete the work set forth in Attachment A — Scope of Services should be <br />specified. <br />PUBLIC WORKS SERVICES DEPARTMENT <br />FY2017-18 STORM PUMP PM AND ON-CALL REPAIR RFP 15 of 23 <br />
