6.D. - Page 51 of 70 <br />ATTACHMENT G <br />WIPHE RESPONSE FORM <br />THIS FORM MUST BE RETURNED WITH SOLICITATION RESPONSE <br />Vendor servicing Washington State AGREES to sell items included in this solicitation to WIPHE <br />institutions at prices offered, unless otherwise noted below: <br />DOES NOT agree to sell to WIPHE Institutions. <br />_ AGREES to sell to WIPHE Institutions at same prices and discounts, with the following <br />exceptions: (attach additional pages as necessary) <br />Vendor must state geographic areas or specific institution(s) listed below within the State of <br />Washington that are EXCLUDED from the contract(s) resulting from this solicitation: <br />Washington Institutions of Public Higher Education (WIPHE). See list on following page. <br />If Vendor agrees to sell to the WIPHE Institutions, a WIPHE Contract number will be assigned and the <br />information will be reported to the Council of Presidents. The Council of Presidents notifies all the other <br />schools when a WIPHE contract has been awarded and a master list will be maintained and the WIPHE <br />website updated.
