The Term of the Original Order, as amended by this <br />Second Amendment, is extended for a period of sixty (60) months from the Effective Date of this <br />Second Amendment (the "Term Extension"). Upon expiration of the Term Extension, service will <br />continue on the same terms as provided on a month-to-month basis at Provider's then current <br />rates unless terminated by either party on not less than thirty (30) days written notice to the other. <br />Section 5: Miscellaneous. Except as expressly amended by this Second Amendment, <br />the Original Order remains in full force and effect in accordance with its terms and is hereby <br />ratified and affirmed by the parties. This Second Amendment may be executed in multiple <br />counterparts, each of which shall constitute an original, and all of which shall constitute one <br />and the same instrument.
