6.D. - Page 60 of 170 <br />Request for Qualifications: <br />General Park Building Construction, Retrofit, Maintenance, and/or Repairs <br />REPORTING REQUIREMENTS AFTER AWARD OF A CONTRACT <br />The contractor is required to provide a Monthly Ethnic Composition of Work Force (ECWF) Report due by the tenth (10'`) <br />of each month for the preceding month. Contractors should submit the original to the Department of Recreation and Parks, <br />Planning, Construction and Maintenance Branch, authorized City representative at the job site. This report must also be <br />submitted by all subcontractors whose contracts exceed Five Thousand Dollars ($5,000.00). <br />The contractor awarded this project will be required to submit a list of all subcontractors on the project prior to commencing <br />work and indicate by an asterisk (*) those whose sub -subcontracts exceed Five Thousand Dollars ($5,000.00). <br />The contractor is reminded that pursuant to the City's Affirmative Action Ordinance, subcontractors whose contracts exceed <br />Five Thousand Dollars ($5,000.00) must submit an Affirmative Action Plan prior to commencing work. <br />The contractor awarded the contract is responsible for the preparation and submission of all reports.
