6.D. - Page 62 of 170 <br />Request for Qualifications: <br />General Park Building Construction, Retrofit, Maintenance, and/or Repairs <br />NONDISCRIMINATION, EQUAL EMPLOYMENT PRACTICES AND <br />AFFIRMATIVE ACTION PROGRAM <br />(CONSTRUCTION) <br />Bidders/Responders/ Respondents are advised that any contract awarded pursuant to this procurement process shall be <br />subject to the applicable provisions of Los Angeles Administrative Code Section 10.8.2., Non-discrimination Clause. <br />Construction projects with the City of Los Angeles for which the consideration is One Thousand Dollars ($1,000.00) <br />or more shall comply with the provisions of Los Angeles Administrative Code Sections 10.8.3., Equal Employment <br />Practices Provisions. All Bidders/Responders/RESPONDENTs shall complete and upload, the Non- <br />Discrimination/Equal Employment Practices Affidavit (two [2] pages) available on the City of Los Angeles' Business <br />Assistance Virtual Network (BAVN) residing at www.labavn.org prior to award of a City contract valued at One <br />Thousand Dollars ($1,000.00)or more. <br />Construction projects with the City of Los Angeles for which the consideration is Five Thousand Dollars ($5,000.00) <br />or more shall comply with the provisions of Los Angeles Administrative Code Sections 10. 8.4 and 10.8.13, herewith <br />referred to as the Affirmative Action Program. All Bidders/Responders/RESPONDENTs shall complete and upload, <br />the City of Los Angeles Affirmative Action Plan Affidavit (four [4] pages) available on the City of Los Angeles' <br />Business Assistance Virtual Network (BAVN) residing at www.labavn.orp- prior to award of a City construction contract <br />valued at Five Thousand Dollars ($5,000.00). Bidders/Responders/RESPONDENTs are required to complete item #17 <br />on page 6 of the City of Los Angeles Affirmative Action Plan Affidavit. Additionally, <br />Bidders/Responders/RESPONDENTs must complete and submit to the awarding department, the Anticipated <br />Employment Utilization Report for each contract awarded prior to issuance of a "Notice to Proceed" to effectuate the <br />requirements of the Los Angeles Administrative Code Section 10.8.13, applicable to construction contracts. <br />Furthermore, the same requirements apply to all subcontractors who must also submit the Anticipated Employment <br />Utilization Report prior to commencing work on the contract. <br />Bidders/Responders/ Respondents shall refer to City of Los Angeles' Business Assistance Virtual Network (BAVN) <br />residing at www.labavn.or�e"Nondiscrimination, Equal Employment Practices and Affirmative Action Program" to <br />complete the Anticipated Employment Utilization Report or visit the Bureau of Contract Administration's web page at <br />http://bca.lacit" to download the form. <br />Bidders/Responders/RESPONDENTS opting to submit their own Affirmative Action Plan may do so by uploading their <br />Affirmative Action Plan onto the City's BAVN.www.labavn.org <br />Both the Non-Discrimination/Equal Employment Practices Affidavit and the City of Los Angeles Affirmative Action <br />Plan Affidavit shall be effective for a period of twelve (12) months from the date it is first uploaded onto the City's <br />BAVN. <br />IMPORTANT — RESPONDERS MUST SUBMIT ALL REQUIRED FORMS (COMPLETELY FILLED OUT) BY RFQ <br />SUBMITTAL DEADLINE. <br />Page 56 of 103 <br />145 <br />
