CEQA: <br />Categorically Exempt - Class 1. Existing Facilities <br />6.C. General services agreement with Bay Area Paving Co., Inc. to repair sidewalks <br />and utility trenches, and install Americans with Disabilities Act -compliant <br />sidewalk ramps <br />Recommendation: <br />By motion, approve and authorize the City Manager to execute a one-year <br />general services agreement with Bay Area Paving Co., Inc. in an amount not to <br />exceed $750,000 for concrete/asphalt repairs, including City Manager authority <br />to extend the agreement for up to two consecutive two-year terms, for a total <br />not to exceed contract price of $3,886,350, and find that the project is exempt <br />from the California Environmental Quality Act (CEQA) under CEQA Guidelines <br />Sections 15301 (Existing Facilities), 15302 (Replacement or Reconstruction), and <br />15304 (Minor Alterations to Land). <br />CEQA: <br />This is not a project under California Environmental Quality Act (CEQA) <br />6.13. Annual self-insured assessment fee for the City's Workers' Compensation <br />Program <br />Recommendation: <br />Adopt a Resolution authorizing the City Manager to pay to the State of <br />California Department of Industrial Relations the City's FY 2018-19 assessment <br />for administering the State of California's Workers' Compensation Program in <br />an amount not to exceed $69,191. <br />CEQA: <br />This is not a project under California Environmental Quality Act (CEQA) <br />6.E. Annexation of 735 Upland Road, Assessor's Parcel Number 058-282-130, to <br />the Oak Knoll Sewer Maintenance District <br />Recommendation: <br />Adopt a resolution consenting to the new sewer service annexation of 735 <br />Upland Road, Assessor's Parcel Number 058-282-130, to the Oak Knoll Sewer <br />Maintenance District (District) subject to the following conditions and approving <br />and authorizing the City Manager to execute an Annexation Agreement with the <br />Property Owner: <br />1.
