General services agreement with Bay Area Paving Co., Inc. to repair sidewalks and utility trenches, and install Americans with Disabilities Act -compliant sidewalk ramps Recommendation: By motion, approve and authorize the City Manager to execute a one-year general services agreement with Bay Area Paving Co., Inc. in an amount not to exceed $750,000 for concrete/asphalt repairs, including City Manager authority to extend the agreement for up to two consecutive two-year terms, for a total not to exceed contract price of $3,886,350, and find that the project is exempt from the California Environmental Quality Act (CEQA) under CEQA Guidelines Sections 15301 (Existing Facilities), 15302 (Replacement or Reconstruction), and 15304 (Minor Alterations to Land). Annual self-insured assessment fee for the City's Workers' Compensation Program Recommendation: Adopt a Resolution authorizing the City Manager to pay to the State of California Department of Industrial Relations the City's FY 2018-19 assessment for administering the State of California's Workers' Compensation Program in an amount not to exceed $69,191.
