COUNTY OF SAN MATEO AGREEMENT WITH INDEPENDENT CONTRACTOR Agreement No. 45000-01-C () {; L Exhlbn "1" Agreement between the County of San Mateo and City of Redwood City Use Additional Sheets as Necessary I. Description of Seryices to be Performed by the Contractor Contractor (City of Redwood City) shall furnish all labor, tools, equipment, transportation, materials, and supervision necessary to provide complete and continuous care of all lawn, trees, shrubs, and ground covers, for ongoing maintenance and other landscaping services at Garfield School Playing Fields as per attached Proposal (Exhibit "B"). Contractor shall submit a cost report to the County for any additional work needed, The County and Contractor shall agree to the scope and cost of any additional work and the additional work shall authorized in writing by the Director of Public Works or his designated representative before said work is perfonned. Contractor shall walk the entire site each month with a County representative to ensure that all operations are being completed in a satisfactory manner. Contractor shall obtain all licenses and permits required by city, county and state authorities, The tenD of this Agreement is for July 1, 2006 to June 30, 2008 a twenty-five month period at the fees quoted on the attached Proposal dated March 27, 2006 (Exhibit "B"). II. Amount and Method of Payment Payment shall be made upon receipt of a written itemized invoice identifying the County project number, complete scope of work, specific work completed, number of hours involved and breakdown of charges, In any event, the total payment for services on Contractor (City of Redwood City) shall not exceed $60,488 (Sixty Thousand, Four Hundred Eighty Eight Dollars and No/OO) attached proposal (Exhibit "B") In any event, the County shall have the right to withhold payment if the County detemrines that the quantity and/or quality of the work perfonned is unacceptable III. Preyailin2 Wa2e When applicable, the Contractor hereby agrees to pay not less than the prevailing rates of wages and be responsible for compliance with all the provisions of the California Labor Code, Article 2-Wages, Chapter 1, Part 7, Division 2, Section l770 et seq,- A copy of the prevailing wage scale established by the Department ofIndustrial Relations is on file in the office of the Director of Public Works, and available at www.dir.ca.govíDLSR or by phone at 415-703-4774, California Labor Code Section 1776(a) requires each contractor and subcontractor to keep accurate payrol\ records of trades workers on al\ public works projects and to submit copies of certified payrol\ records upon request.
