Notification shall include the reason and the possible <br />duration of the absence. <br /> <br />Employees are responsible for keeping the Department informed of <br />their continuing condition and probable date of return to work. <br /> <br />Upon return to duty, the absent employee must complete an <br />absent report and forward it to the Fire Chief, stipulating the <br />reason for the absence and the number of hours of sick leave use. <br /> <br />For absences in excess of three (3) calendar days, the Rre Chief <br />may require a physicianrs statement from an employee who applies <br />for sick leave, or whatever investigation into the circumstances <br />which appear warranted, before taking action on the request. <br /> <br />To ascertain the propriety of claims, the following procedure shall <br />apply: <br /> <br />Employees who notify the Battalion Chief's office of their inability <br />to report for duty shall report to the Battalion Chiefs office where <br />they can be reached. If the employee will be unavailable for a <br />telephone call, the employee shall notify the Battalion Chief, or if <br />he/she is not available, the Dispatch Center, stipulating the reason <br />and length of time of such unavailability. <br /> <br />An employee who is telephoned at the designated number <br />between 0800 and 2100 hours and. who fails to answer the call, <br />shall not be paid, unless the employee has notified the Battalion <br />Chief, as provided above. <br /> <br />Abuse of sick leave by an employee wili subject the employee to <br />disciplinary action. <br /> <br />18.2 Umited Duty Work For An Injured Employee <br /> <br />18.2.1 <br /> <br />18.2.2 <br /> <br />An injured employee who is receiving medical attention for an <br />alleged industrial injury, and who is determined to be temporarily <br />disabled, is required to return to the department after initial <br />treatment with a medical report indicating what, if any, limited <br />duty the employee can perform. If none, the report should so <br />indicate. The department routinely requires periodic updated <br />reports, which should also contain limited duty information. <br /> <br />An employee on extended sick leave is likewise required to provide <br />these periodic reports, which should also contain limited duty <br />information. <br /> <br />34 <br />
