<br />6.10 <br />Page 52 <br /> <br />or taxes required of the City under the federal or state act, shall exceed the <br />amounts paid at the time such legislation becomes effective. <br /> <br />If the benefits provided under the federal or state act exceed the benefits <br />provided hereunder in each category of coverage, the City shall be under no <br />further obligation to make any contributions. <br /> <br />19.4 Life Insurance <br /> <br />Effective within sixty (60) days of ratification and council approval of an <br />agreement, the City shall offer to eligible employees additional life insurance <br />equal to one and one.half (11/2) times the employee's annual salary, at a 60/40 <br />premium contribution split between the City and the employee respectively. <br /> <br />19.5 Vision Care <br /> <br />Effective the 1st of the month directly following ratification and Council approval <br />of agreement, the City shall contract with Vision Service Plan (VSP) or a <br />comparable vision care provider to provide Vision Care Plan B benefits for <br />LA.F.F. employees and their dependents. The Vision Service Plan B provides <br />for an exam every 12 months, lenses every 12 months if needed, and frames <br />every 2 years if needed. There is to be no deductible for employees, but a <br />$20.00 per person deductible will apply to dependents each time benefits are <br />available and will be paid by the employee. <br /> <br />19.6 Long Term Disability Insurance <br /> <br />Effective with the first full pay period directly following the ratification and <br />Council approval of an agreement, the City will reimburse sixteen dollars <br />($16.00) per month per employee towards the cost of Long Term Disability. <br /> <br />In the event the cost of the current Long Term Disability plan increases during <br />the term of this Memorandum of Understanding, the City will reimburse the <br />actual premium cost up to a maximum of twenty-five ($25.00) per month per <br />employee. <br /> <br />SECTION 20 - SAFETY <br /> <br />Both the City and the Union shall expend every effort to see to it that work is performed <br />with a maximum degree of safety, consistent with the requirement to conduct efficient <br />operations. <br /> <br />Each employee covered by this MOU agrees to comply with all safety rules and <br />regulations in effect and any subsequent rules and regulations that may be adopted. <br />Employees further agree that they will report all accidents and safety hazards to the <br />appropriate management official immediately.
