<br />Exhibit F <br />HUD Required Quarterly Reports <br /> <br />City of Redwood City - Fair Oaks Community Center <br />Core Services FY 2006-07 <br /> <br />No request for reimbursement will be honored until all reports required hereunder have been <br />received by the Department of Housing. <br /> <br />Contractor shall submit to the Human Services Agency the following reports within 30 days <br />of the end of each quarter: <br /> <br />1. Performance Log (optional), to be submitted on form provided by the County Human <br />Services Agency (HSA), which shall provide income and demographic information of <br />each individual or household served as well as a brief description of the services <br />provided during the quarter. <br /> <br />2. Performance Summary, to be submitted on form provided by the County HSA, which <br />shall summarize the number of clients served by their ethnicity and income. <br /> <br />3. Consolidated Funding Source Report. <br /> <br />4. Ouarterly Proiect Narrative Report, which provides a narrative of highlights/details of <br />both the funded activity and of the Contractor's organization. <br /> <br />Performance reporting forms will be provided to Contractor by the County of San Mateo <br />Human Services Agency Electronically. <br />
